You can book a Polypharmacy Review with one of our Health Board Pharmacists through the Pharmacy Hub. These reviews are designed to help ensure your medicines are working well for you and that you are getting the most benefit from them.

Your appointment will take place by telephone. A pharmacist will review your medications with you and answer any questions you may have about your treatment.

Once you have booked an appointment, the pharmacist will call you on your selected day between 2:00pm and 4:30pm. Please ensure you are available to take the call during this time.
