To request an on-the-day telephone appointment, use the form below. A clinician will contact you in the morning (AM) or afternoon (PM), depending on your choice. Please keep your phone nearby and be ready to answer, as the call may come from a withheld number.

You will be contacted by a clinician; this may not be a GP, and could be an Advanced Nurse Practitioner or another qualified member of our clinical team.

When completing the form, please provide as much detail as possible about the reason for your appointment — this helps the clinician prepare before calling you.

This service is for telephone consultations only. If the care advisor decides that a face-to-face appointment is more appropriate, they will contact you to arrange this. If you require a face-to-face appointment directly, please contact the practice in the usual way.

The form is available daily between 5:00am and 8:00am. If there are no appointments available, please try again the next working day or contact the practice as normal.
