If you’re here, it means the clinical team at Barclay Medical Practice has invited you to attend an annual health review.

This is because we want to check in on your condition and make sure you’re receiving the right care from us, whether that might involve adjusting your medication or keeping track of any symptoms you’re experiencing.

Your annual review will consist of a blood test that we carry out to monitor your condition and detect any changes. To get started, just fill out the short form below to let us know when you’re available.

Our clinical team at Barclay Medical Practice actively monitors and reviews all online annual review submissions. Once you send yours in, one of our team will check your response and reach out by phone or text to confirm your appointment.

Annual reviews—like this one you’ve been invited to—are here to help you manage conditions such as high blood pressure, heart disease, vascular disease and other cardiovascular conditions. These are long-term conditions that can sometimes progress silently. Regular monitoring is the only reliable way to help us detect any changes early, ensure your current treatment remains effective and make important adjustments to your treatment plan, if necessary.

During your review, a Barclay clinician will carry out some essential checks, including blood tests, blood pressure measurements and other relevant assessments. This allow us to track your cardiovascular health and identify any potential complications. If any concerns arise from your results, we’ll arrange a follow-up appointment with you to discuss the next steps.

Even if you’re feeling well, it’s important to attend your annual review. Many vascular conditions don’t cause noticeable symptoms until later stages, which means early detection can make a significant difference in preventing serious health issues.

An annual review is just like a normal blood test. The healthcare professional who arranges your annual review will tell you whether there are any specific instructions you need to follow before your review.

At the appointment, a healthcare professional will take a small amount of blood, usually from the inside of your elbow. We’ll send this blood sample to a laboratory, where it will be examined under a microscope or tested with chemicals, depending on what’s being checked.

The results are sent electronically back to our practice. One of our clinicians will interpret the results and assess whether they’re normal or whether they may require further action.

Some test results may be ready within a few days, although others may not be available for a few weeks. 

If you’re worried about needles, or you have a history of feeling lightheaded during and after blood tests, give us a call to let us know, or tell the healthcare professional carrying out your review on the day so that they’re aware and can help you feel more comfortable. You can also bring someone with you to the annual review to support you.

You may be taking heart medication as part of your current treatment plan. There are many different medicines for vascular conditions. Sometimes they can look similar, but there are often small differences in how they work and how they help manage your symptoms.

You may be prescribed a combination of multiple medicines, or we might update what medicines you take if there are any changes to your condition.

If you take heart medication, it’s important to:

  • follow your treatment plan carefully by taking your medicines as prescribed by our clinical team
  • tell us about any other medications you’re taking
  • discuss any side effects or a change in symptoms with us as soon as possible
  • talk to a member of our clinical team or your local pharmacy if you need help remembering to take your medication
  • read the patient information leaflet that comes with your medicine—if you’re worried about anything, discuss this with a member of our clinical team

Do NOT stop taking your prescribed medicines without medical advice—this could make your condition worse.

If you are pregnant or breastfeeding, tell our clinical team. This means we can prescribe the most suitable and safest heart medication for you, as there are some medicines pregnant or breastfeeding people should not take.
